An Ivy pole
Chained to something holding you back, chained to a pole with no means to detach. Living a life of captivity by yourself, compelled to say "it's for the betterment of myself". Sympathy, empathy he receives it all, but that does not make it better for him at all. When will people understand? that he is not sick, he is not broken, he is not a patient but just a boy with an ivy pole attached to his hip. A boy longing to live a moment outside these hospital walls that is making him sick. As he reached the fourth stage, there was no going back there were no means of living to see another day, and the pole is never going to detach.
